Our Case Managers are from a variety of backgrounds – from nursing and speech-language pathology to occupational therapy, social work, and everything in between. Advocating for our clients is at the core of what we do. We stand by their side, ensuring they receive the best care and support available. - You’ll enhance the quality of the client’s experience by leveraging your knowledge of the medical system and partnering with healthcare providers to ensure clients receive seamless, all-encompassing care.
- Educate and empower clients and their family members to equip them with the resources and assistance they need to conquer their health challenges.
- Identify and lead clients to community resources and the support they need to thrive.
- Coordinate client care with the assistance and direction of the physician, assess treatment plans and keep impeccable client records (treatment plans, case notes, discharge plans, etc.).
- Partner with healthcare providers and fellow teammates to make sure every client gets the care they deserve. Attend important client appointments and advocate for them.
- Assist with hospital discharges as needed, assuring they return to a safe and functional home environment. Establish treatment programs by setting schedules and routines; coordinate services being provided; and arrange logistical details such as transportation.
- Monitor client’s rehabilitative progress by verifying attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements.
- You have an active professional license and at least 1+ year experience in medical management, medical/team collaboration within a healthcare system/setting with a degree in one of the following areas: nursing, case management, physical therapy, speech and language pathology, social work, respiratory therapy and/or occupational therapy.
- You have the essentials – a schedule to maintain flexibility, working knowledge of MS Office, laptop, cell phone and reliable transportation.
- You have an unmatched level of empathy, offering clients a reassuring hand to hold onto during their medical journey.
- Your verbal and written communication skills are clear and articulate, especially when writing reports and interacting with healthcare providers and attorneys.
- You’re extremely organized, which is essential in arranging client notes and records, to ensure the highest level of care is provided.
- You are guided by a strong ethical compass, respecting client confidentiality in alignment with HIPAA laws.
